Confidentiality:
- As a licensed health care practitioner, I, Noelle Tabor, LMT, have a “duty to warn” under the following circumstances: if I believe you are a danger to yourself or others; if I believe or know that you are abusing a minor, a senior, or a disabled or otherwise impaired person; or if I become aware of past abuse of a child that has not been previously reported to the authorities.
- Sessions are strictly confidential. In compliance with Federal and State laws, I will only share your confidential information if I have your signature on a qualifying document or by court order.
- If you would like me to share details of your case with other professionals, please write their name(s) on the designated line of the intake form you will fill out prior to your session.

Professionalism:
- I, Noelle Tabor, LMT, subscribe to a code of ethical behavior, which is available upon request. I follow all of the statements in this ethical code.
- I respect all clients regardless of their age, gender, race, national origin, sexual orientation, religion, socio-economic status, body type, political affiliation, state of health, and personal habits.
Payment Policies, Session Fees, and Recourse:
- For first-time clients, payment is due at the time of booking.
- I accept payment through the booking platform massagebooks.com where current session fees can also be found.
- I do not take insurance at this time. However, I do accept Musiccares. Please make sure you have an approved Musiccares grant before booking an appointment.
- I do not accept tips.
- If, during my assessment, I determine with reasonable certainty that my work cannot help you, we will end the session at that time, and you will not be charged for the initial appointment.
- In case of dissatisfaction with a session, clients are entitled to a full refund or a complimentary treatment. However, refunds will not be issued if a session is halted due to a client’s behavior compromising the therapist’s safety or if the therapist suspects the client is under the influence of drugs or alcohol.
Cancellations, Late Arrivals, and No-Shows:
- Non-emergency cancellations made with less than 12 hours’ notice will be charged in full for the missed session.
- For emergencies, I understand life happens—these situations will be handled with care and compassion on a case-by-case basis. Please know there’s always room for grace in my practice.
- Arriving more than 15 minutes late without communication will be considered a no-show, and the session will be charged in full.
- If a session begins late due to client tardiness, it will end at the originally scheduled time. However, if any delay is on my part, you will receive the full session time, or your fee will be adjusted to your benefit.
- If I must cancel, I will notify you at least 24 hours in advance whenever possible. If unable to do so, your next session is on me.
Treating Minors:
I will request a parent or guardian be present during the session for children under 16. All individuals under 18 must have signed parental permission.
Informed Consent:
Prior to each session, the treatment plan will be discussed with you. Prior to your first visit, you will be asked to sign the treatment consent form stating that you have read the information, understand it, and agree to comply with the policies and procedures. Clients may be asked to update this information on a yearly basis.
